2010 Statewide Math and Science Competition- Registration Info

MATH/SCIENCE REGISTRATION INFORMATION

Please review all the information below to ensure that your group will be able to participate in the 2010 Math/Science competition.  Only partners and certified chapters are eligible.

STEP ONE: Due February 1st

IDENTIFY YOUR chapter'S CONTEST COORDINATOR AND SUBMIT INTENT TO PARTICIPATE FORM ONLINE

Each chapter is required to have one M/S "Contest Coordinator".  This person will act as the group liaison and is responsible for the coordination of his group's travel arrangements, chaperones, hotel rooming list, permission slips, other required forms, etc.  The Contest Coordinator MUST attend the M/S Contest with their chapter. 

Once a coordinator is identified, he/she will need to review the Contest Coordinator Checklist and submit an Intent to Participate form by February 1st. Once the state office receives your “Intent to Participate" all future information regarding the contest will be sent directly to your contest coordinator, including the group registration form, rooming list template, hotel information, etc.  It is their responsibility to forward email communication to other local volunteers as they deem necessary.

STEP TWO: Due Friday, March 19th at noon

SUBMIT REGISTRATION FORM VIA EMAIL

The 2010 registration form will be sent via email to the chapter's Contest Coordinator after we receive the Intent to Participate form.  The Registration Spreadsheet must be completed in the Excel format and submitted via email to the TAME office by March 19, 2010.   (Form cannot be accepted via fax). A rooming list template will be provided once the registration spreadsheet has been received.

STEP THREE: Due Thursday, March 25th at noon

SUBMIT ROOMING LIST VIA EMAIL

A rooming list template will be provided once your completed registration spreadsheet has been received. The rooming list must be submitted via email to the TAME office by March 25, 2010.

STEP FOUR: due upon arrival at contest registration  

Demographic form

Permission forms must be provided for EACH student and must be signed by their parent/guardian. The state office needs copies to keep, while you will need the originals with you as you travel to and from the competition.  Each student MUST provide you with one BEFORE he/she can participate. Please provide copies of all permission forms at registration.

T-shirt Request form - to be turned in at registration based on the final number of students and chaperones in attendance.
