MATH/SCIENCE REGISTRATION INFORMATION Follow the checklist below to make sure your group will be able to participate in the 2008 Math/Science competition. Only certified alliances are eligible.
IDENTIFY YOUR "ALLIANCE CONTEST COORDINATOR" AND SUBMIT "INTENT TO PARTICIPATE" FORM ONLINE Each Alliance is required to have one M/S Contest Contact person, or "Contest Coordinator", for their Alliance. This person will act as the group liaison and is responsible for the coordination of his group's travel arrangements, chaperones, hotel reservations, permission slips, required forms, etc. This person MUST attend the M/S Contest with their alliance. Once a coordinator is identified, he/she will need to fill out the form called "Intent to Participate" by January 31st so we can plan the approximate number of attendees for the competition, allocate funding and arrange for the appropriate number of hotel rooms, volunteers, space requirements, meals, shirts, etc. Once the state office receives your “Intent to Participate," all future information regarding the contest will be sent directly to your alliance coordinator, including the group registration form, rooming list template, hotel information, etc. It is their responsibility to forward email communication to other local volunteers as they deem necessary. SUBMIT REGISTRATION FORM VIA EMAIL The 2008 registration form will be sent via email to the Alliance Contest Coordinator after we receive the Intent to Participate form. Registration spreadsheet must be completed in the Excel format and submitted via email to the TAME office. (Form cannot be accepted via fax)
Permission slips must be provided for EACH student and must be signed by their parent/guardian. The state office needs copies to keep, while you will need the originals with you as you travel to and from the competition. Each student MUST provide you with one BEFORE he/she can participate. T-shirt Request form - to be turned in at registration based on the final number of students in attendance. |
