MATH/SCIENCE REGISTRATION INFORMATION Follow the checklist below to make sure your group will be able to participate in the 2005 Math/Science competition.
IDENTIFY YOUR "ALLIANCE CONTEST COORDINATOR". Each Alliance is required to have one M/S Contest Contact person, or "Contest Coordinator", for their Alliance. This person will act as the group liaison and is responsible for the coordination of his group's travel arrangements, chaperones, hotel reservations, permission slips, required forms, etc. This person MUST attend the M/S Contest with their alliance. SUBMIT "INTENT TO PARTICIPATE" FORM ONLINE Once a coordinator is identified, he/she will need to fill out the form called "Intent to Participate" by March 15th so we can get an idea of number of attendees for the competition and allow for the appropriate number of hotel rooms, on site volunteers, space required, etc. SUBMIT REGISTRATION FORM VIA EMAIL ONLY The 2005 registration form will be sent via email to the Alliance Contest Coordinator as soon as we receive the Intent to Participate form. Registration is NOT complete until all sheets on the registration form are complete and turned into the state office.
Demographic form: Coordinator will need to fill out and turn in at registration. Permission slips must be provided for EACH student and must be signed by their parent/guardian. The state office needs copies to keep, while you will need the originals with you as you travel to and from the competition. Each student MUST provide you with one BEFORE he/she can participate. T-shirt Request form - to be turned in at registration based on the final number of students in attendance. |
